Find A Designated Civil Surgeon
If you have online access to the World Wide Web, you can find a selection of qualified doctors on the USCIS Civil Surgeons Database.
Using a telephone, you can receive the names of USCIS-designated Civil Surgeons in your area by calling the USCIS National Customer Service Center at 1 (800) 375-5283. You will need to provide your zip code in order to hear a list of Civil Surgeons nearest to you. When you call, be sure to have a pen or pencil ready to write down the names and telephone numbers as they are recited.
Information regarding the medical exam procedure can be found online at the USCIS website and on the form instructions.
PLEASE TAKE NOTE: At your medical exam, be sure to
- BRING the I-693 Medical Examination form and the I-693 Supplemental Form with you, and
- BRING to the USCIS-designated Civil Surgeon any past TB skin test results, if you have them.
