Finding A Designated Civil Surgeon in Your Area

If you have online access to the World Wide Web, you can find a selection of qualified doctors on the USCIS Civil Surgeons Database at: https://egov.immigration.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV. At your local USCIS office, a list of Civil Surgeons is also available.
Using a telephone, you can receive the names of USCIS-designated Civil Surgeons in your area by calling the USCIS National Customer Service Center at 1 (800) 375-5283. You will need to provide your zip code in order to hear a list of Civil Surgeons nearest to you. When you call, be sure to have a pen or pencil ready to write down the names and telephone numbers as they are recited.

PLEASE TAKE NOTE: At your medical exam, be sure to
· BRING the I-693 Medical Examination form and the I-693 Supplemental Form with you, and
· BRING to the USCIS-designated Civil Surgeon any past TB skin test results, if you have them.
